Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Our doctor routinely performs surgery on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. We will certainly do the surgery on your first visit if there are no contradictions. After all, we want to get you back to your normal daily activities.
REQUIRED FORMS
The purpose of these forms is to assist us in providing you with the highest level of care. We understand that you may have completed similar forms at your dentist’s or physician’s office, however we do ask that you take your time to carefully read and accurately fill out these forms. If you have any questions, please do not hesitate to ask for assistance.
The following forms must be completed prior to your appointment. You can download the forms by selecting the “Online Forms” link in the left side menu. Having these forms filled out before you arrive will save you a lot of time and will shorten your wait time to see the doctor.

MEDICAL HISTORY
Those taking medications should bring the name and dosage of the drug(s) at your appointment time. Having all of this information shortens your wait time and can possibly mean that your surgery can be done the same day.
INTRAVENUS SEDATION
If you prefer I.V. sedation (sleeping) for the surgery, refrain from the intake of food or liquid (even water) for six hours prior to the appointment. A responsible adult must accompany you to drive you home and remain with you for several hours or until fully recovered from the sedation. Medications prescribed by your physician should be taken with a small amount of water. If you take insulin or pills for diabetes you must have direct communication with our doctor prior to changing any medicine regime.
INHALATION SEDATION
If you prefer nitrous oxide (laughing gas) for sedation for the surgery, limit your oral intake to a light meal up to two hours prior to the procedure. Medications prescribed by your physician should be taken unless directed otherwise.
REFERRAL SLIP AND X-RAYS
Patients referred by a dentist should bring the referral slip and x-rays at your appointment time. In some cases x-rays may be inadequate and will be retaken in our office. You do not have to be referred to our office by another doctor or dentist. We accept patients directly so you feel free to call to set up an appointment.
MINORS
A parent or guardian must accompany any patient under the age of 18.
SCHEDULING
We strive to “stay on time” in the scheduling of patients. However, because of the nature of oral surgery, delays are sometimes unavoidable.
